MoCA score of 22/30 is below the clinical threshold of 26. An ASWP referral has been prepared with patient consent.

The referral packet contains zero PHI. Only the internal referral ID is transmitted. ASWP retrieves patient-authorized information via a separate secure FHIR handshake.
